Thank you for participating in a Jennifer Oliphant Photographer photo session. Please fill out the form and submit below once you have read the terms and conditions.

I hereby grant permission to Jennifer Oliphant Photographer and the business/company Jennifer is photographing/filming for to use photographs and video content of my child in print, social media website content, and any other advertising purposes. I acknowledge Jennifer Oliphant Photographer’s right to crop or treat the photograph/video at its discretion. In addition, I waive any right to inspect or approve the finished product, including written copy, wherein my child’s likeness or comments appear.
I also acknowledge that Jennifer Oliphant Photographer and the business/company Jennifer is photographing for may choose to use my child’s photo/video/comments at this time, but may do so at its own discretion at a later date.
Jennifer Oliphant Photographer and the business/company Jennifer is photographing for reserves the right to discontinue the use of photos/videos/comments without notice.
I understand that I am responsible for my own and my child’s actions on the photographer’s premises and on location.
